Leveraging Palliative Care for Seamless Transitions

Ensuring patients with a life-limiting illness have the right care at the right time is essential in meeting their needs. With increased regulatory scrutiny on hospice eligibility and length of stay, hospice organizations must leverage partnerships with palliative care organizations to thrive. Providing palliative care for patients prior to them becoming hospice eligible is the best way to address symptom management, goals of care discussions and advanced care planning. Experts share how to best manage palliative care patients who are approaching end of life to ensure an easier transition to hospice. They will also discuss how patients admitted to hospice who develop a prolonged prognosis benefit from seamless transitions to a trusted palliative care program, and explore creative collaboration, technology-enhanced tools and messaging to the patient and family to ensure quality patient care and improved patient satisfaction.

Speakers:

- Sarah A Payne, DO, HMDC, Medical Director, Banner Hospice

- Rory Farrand, Vice President, Palliative and Advanced Care, National Hospice and Palliative Care Organization

- Amy Rose, MSN, RN, CHPN, Axxess
