Leveraging Palliative Care for Seamless Transitions
Ensuring patients with a life-limiting illness have the right care at the right time is essential in meeting their needs. With increased regulatory scrutiny on hospice eligibility and length of stay, hospice organizations must leverage partnerships with palliative care organizations to thrive. Providing palliative care for patients prior to them becoming hospice eligible is the best way to address symptom management, goals of care discussions and advanced care planning. Experts share how to best manage palliative care patients who are approaching end of life to ensure an easier transition to hospice.
