SCAN Health Plan Rolls Out Home-Based Integrated Care Model

SCAN Health Plan, subsidiary of SCAN Group, recently launched a new integrated care delivery model focused on seniors who are home-bound or reside in assisted living and skilled nursing facilities. Members of the company’s Embrace Medicare Advantage plan are eligible to participate in the model, which integrates primary care, geriatric and palliative care medicine.

Through the model, SCAN will deliver clinical care typically provided in physician offices and hospitals to patients wherever they reside. The model gives patients with complex needs greater access to a multidisciplinary team aimed at delivering a comprehensive care, including nurses, physicians, pharmacists, care coordinators, social workers, among other professionals, according to the company.

Enrollment in the Embrace Institutional Special Needs Plan for calendar year 2022 will begin Oct. 15.

“By providing care directly to our members where they live, we are better positioned to understand and address their complex medical and social needs,” said Sachin Jain, M.D., president and CEO of SCAN Group and SCAN Health Plan. “Our new Embrace plan and mobile care model is just one example of how SCAN is evolving to deliver on our mission to keep older adults healthy and independent.”

California-based SCAN Health Plan covers more than 220,000 patients across the state, with plans to move into Arizona and Nevada in 2022. The $2.4 billion organization is a part of the SCAN Group, a nonprofit dedicated to helping senior citizens stay healthy and independent.

“Our new model provides members with a single point of contact to navigate all of their needs and take advantage of the wide array of benefits our plans offer,” said Samantha Roushan, vice president of integrated care delivery and innovation at SCAN.

SCAN built out its home-based care capabilities last month when it invested in MedArrive. The company is a health care logistics and services platform that enables payers to clout emergency medical services (EMS) and other skilled providers to enhance home-based care, including palliative care. Services include chronic condition management, transitional care, readmission prevention, urgent care, palliative care, and physician-led telehealth, among others.

SCAN’s Embrace model falls in line with a growing trend of health care moving into the home. This trend gained momentum during the pandemic as more patients tried to avoid facility-based settings.

The U.S. Centers for Disease Control and Prevention (CDC) estimated that roughly 41% of adults nationwide had avoided or delayed medical care last year, including 32% who avoided routine care and 12% who avoided urgent and emergency care. This adversity could lead to a rise in morbidity and mortality associated with both chronic and acute health conditions, according to the CDC.
