Transitions Hospice has expanded into Michigan with a de novo in the Troy community. The new location will care for patients in Detroit, Lansing, Ann Arbor and a number of surrounding communities. The move furthers Transitions’ multi-state expansion plan, focused on Midwest markets.
Transitions Hospice is a portfolio company of the Transitions Group, which also holds skilled nursing, home health, medical equipment and therapy assets. Prior to the Michigan de novo, the company’s footprint was concentrated in Illinois and Indiana.
“I believe our rapid growth and entry into Michigan is a reflection of the dedication that our teams bring to each patient they serve,” said Transitions CEO Jim Palazzo. “I am excited to show our passion for hospice to communities in Michigan. We commit to hospice with the goal of making a difference in not only our patients’ lives, but the lives of their families as well.”
Michigan had the 10th highest rate of hospice utilization among Medicare decedents during 2018 at 53.8%, according to the National Hospice & Palliative Care Organization. The national average that year was 50.3%. Utah led the nation with 60.5%.
Demographic tailwinds run strong in Michigan where 26% of the population are 60 or older, according to the U.S. Census Bureau.
St. Croix Hospice, a portfolio company of the private equity firm H.I.G. Capital, also recently entered the Troy, Mich., market with the acquisition of CNS Hospice for an undisclosed amount.
Transitions is no stranger to growth. The company opened new locations in Springfield, Ill., and northwest Indiana during 2020. The hospice expects to stretch into neighboring states in addition to a possible push toward the East through a mix of acquisitions and de novos.
