Perfect Health, Concerto Merge, Offer Palliative Care

Senior care provider Perfect Health is merging with home care company ConcertoHealth. The combined operation, branded as ConcertoCare, is backed by the private equity firm Deerfield Management Co., and will offer community-based palliative care in addition to a range of other services. 

ConcertoCare is a tech-enabled, risk-based integrated care company that provides palliative care, home-based primary care, PACE and programs to address social determinants of health. The organization uses a proprietary technology platform, Patient 3D, to analyze patient data and identify social needs, formulate care plans and drive quality improvement.

“Perfect Health built a clinical model specifically designed to support older adults, challenged by aging and illness, to continue to live safely in their homes,” ConcertoCare Chairman and CEO Julian Harris,. M.D., told Hospice News. “ConcertoHealth had already developed their ability to utilize proprietary technology to identify and help direct and deliver care for those most in need in their communities. We saw that the melding of these approaches would enable the combined organization, ConcertoCare, to create a best-in-class model of community-based care in the home for seniors.”

The company’s business model centers around its ability to partner with Medicare Advantage and parties with dual-eligibility through risk-bearing, value-based contracts. This supports a comprehensive geriatric care model.

ConcertoCare operates according to a three-fold model designed to improve patient outcomes and quality of life as well as reduce health care costs. The company reports having reduced hospital admissions among its patient population by 47%, readmissions by 40% and emergency department visits by 16% while increasing by 38% the number of senior wellness visits.

The model consists of services for patients who have complex care needs and an existing primary care provider, a home-based and virtual primary care program and a soon-to-be added PACE operation.
